If you need to make a payment for anesthesia services at MPS Surgical Suites you have three options:

1). Click the “Pay Now” button below, or use the camera on your mobile device to scan the above QR Code. Both payment options will take you to our Square Up payment account. You will need to enter the amount you are paying.

2). Open a PatientFi Line of Credit, or authorize existing PatientFi LOC funds to pay Providian Anesthesia.

3). We accept cash, Cashier’s Checks or Money Orders made payable to Providian Anesthesia LLC.  If you would like to pay by cashier’s check or money order please mail to: Providian Anesthesia, 12468 Lagrange Rd. Suite 342, Louisville, KY 40245. Make checks payable to Providian Anesthesia, and mail your payment at least two weeks prior to your procedure date. Payment not received by your procedure date will need to be reconciled by credit or debit card, and your check will be returned once received. If you would like to pay by cash please call (502) 208-1191 to arrange in-person payment.

If you have a question regarding your anesthesia bill, or prefer to pay over the phone, please call 502 208-1191 to speak with a Providian Anesthesia representative. No personal checks allowed.

Bluegrass Surgery and Laser

If you need to make a payment for anesthesia services at Bluegrass Surgery and Laser please click the “Make payment” button above. If you have a question regarding your anesthesia bill for anesthesia services at Bluegrass Surgery and Laser Center, please call 800 - 951 - 7850. This will put you in contact with our billing group who are best able to answer billing questions and take payment.

We at Providian Anesthesia understand medical bills can sometimes be financially challenging. If you would like to inquire about extended payments please speak to the billing representative about payment plan options.
